Pharmacological treatment

Analgesics for pain management
Ibuprofen (PO) 400mg 8hourly for 3 days OR Diclofenac (IM) 75mg stat then continue (PO) 50mg 8hourly OR Tramadol (IV) 100mg stat then continue with (PO) 50mg 8hourly OR Pethidine (IM) 100mg stat

Antiemetics in case of vomiting

IV fluids

For distal ureteric calculi less than 7mm
Tamsulosin (PO) 0.4mg 24hourly for spontaneous stone expulsion.

In case of calcium stones Thiazide diuretics are recommended
Bendrofluazide (PO) 5mg 24hourly OR Hydrochlorothiazide (PO) 12.5-50mg 24hourly.

In case of uric acid stones ,allopurinol (PO) 50-100mg 24hourly is recommended.

Surgical treatment

Surgery is indicated in case of Obstructive uropathy and infection. It can be done as uteroscopy, percutaneous nephrolithotomy or extracorporeal shockwave lithotripsy.

Percutaneous nephrostomy placement or ureteric DJ Stenting is recommended for emergency decompression.
