Surgical anatomy of prostate gland

Prostate gland is a walnut shaped gland located around the urethra at the base of the penis. It is an accessory male reproductive structure which produces an alkaline fluid which nourishes and transports sperm.

Parts

Central zone, surrounds the ejaculatory ducts, making about 25% of normal prostate volume.

Transitional zone, located centrally and surrounds the urethra, making about 5-10% of normal prostate volume.

Peripheral zone, makes up the main body of the gland (about 65%) and is located posteriorly. The peripheral zone is mainly the area felt against the rectum on DRE.

Fibromuscular stroma, situated anteriorly in the gland. It merges with the tissue of the urogenital diaphragm.

Vasculature

Prostatic arteries supply the prostate which originate from internal iliac arteries. Some branches arise from internal pudendal and middle rectal arteries.

Venous drainage is via  prostatic venous plexus, draining into the internal iliac veins.
