Maternal resuscitation
Apply the ABCD protocol.
Insert two (2) large bore IV lines and give Normal Saline/Ringers Lactate. Replete 3 times the ongoing blood loss.
Blood Transfusion based of patients Hb level or clinical degree of anaemia. Oxygen therapy at a flow of 6L/min.
Insert a urinary catheter to monitor input/output
In case of Disseminated Intravascular Coagulation (DIC), Give fresh frozen Plasma 1 Unit/hour otherwise give whole blood based on the lost amount of blood.
Obstetric Management
If the foetus is viable perform emergency Caesarean section
If the foetus is not viable, Normal vaginal delivery is preferable in absence of contraindications.
Prophylactic antibiotics
Amoxycillin + clavulanic acid (FDC) (IV) 1.2 g stat
AND
Metronidazole (IV) 500mg stat.
